The overall goal is to provide full educational opportunities to all children with disabilities. The policy of the Board of Elementary and Secondary Education reaffirmed that free and appropriate educational opportunities shall be made available to all children with disabilities between the ages of 0-21.

    • Using our Customized Scan Service, you can choose the questions you need. Each questionnaire form has a total of 100 lines available up to a length of 90 letters and spaces each with five response categories.

    Demographics Analyzer

    We can help you with educational equity* reports. You can use a special codes option (demographics) at the top of the questionnaire to enter up to 20 spaces for data about students, staff, etc. Our program can then analyze your questionnaire results by those demographics and print the reports you need.

    Student Demographics could include:
    Gender - Male, Female
    Race - White, Black, Hispanic,
    American Indian, Asian
    Status - Handicapped, Migrant, Economically
    Deprived, LEP
    Teacher - Name
    		Staff Demographics could include:
    Gender
    Race
    Position (superintendent, principal,
    Grade - K-2, 3-6, 7-9, 10-12 teacher, etc.)

     *Educational equity is the product of a process which ensures fairness for every child within the educational system.

    The final report shows you, by response group, each question in the survey, the percentage of responses for each question, total responses for each question and overall total response and percentage. A sample report is attached.
